Proof of Purchase Information

Replacement: Proof of purchase is the end user's sales receipt or paid or zero balance invoice that shows that they purchased an LP or LPG appliance.

New Construction: Must submit a purchase contract/builder contract with the homebuyer, bill of sale, or builder contractor's work order with the installation address. A Texas Gas Check Form or Gas Appliance System Check Form also will be accepted. General Gas Check forms that do not contain propane appliance information will not be accepted.
